This Phase 4 pragmatic study provides a connected home blood pressure cuff and pairs participants with a clinical pharmacist who adjusts medications remotely in partnership with the participant's own doctor.
Study objective
To compare the proportion of participants reaching a blood pressure target below 130/80 at 6 months.
